I give consent to the Kids Camp team to use pictures/videos of my child(ren) taken at the event for internal promotional materials only. No photos will be posted on social media or the internet.

Our camp staff cares about campers and will take appropriate measures to ensure their safety.
As the parent or legal guardian, I give permission for the participant(s) named above to attend SCUBA Kids Day Camp, July 1 to 5, 2024. I understand that there are certain risks of injury inherent to the activities at day camp, and I am willing to assume these risks on behalf of my child(ren).

In addition to giving my full consent for my child(ren) to participate, I do waive, release and hold harmless Lake Country Alliance Church, it's officers and all volunteers, for any injury that may be suffered by my child(ren) in the normal course of participation in these activities.
